Isolation Meal Pack Request Form

Fill out and submit this Isolation Meal Pack Request Form (PDF) to make your request.

This form needs to be submitted by 11:59 PM for the next day.

You will get one package of food daily containing the following:

Fruit Juice OR Gatorade
Granola bar--Jello--Fresh Fruit
Soup of the Day/Crackers
Sandwich of the Day
Cereal/milk OR Oatmeal

This pack is planned to feed you for a day. Sorry no substitutions.

Two meal swipes will be charged to your meal plan for this meal pack. You must arrange to have a friend pick up your meal. Meals will be available for pick up between 11:00 AM and 2:00 PM.

Please complete a new form for each day you are in isolation, unable to attend the Marketplace and would like a meal pack. If you run out of meal swipes, Hawk Dollars or U Bill Charge will be used.

Residents in Burge, Daum, Currier, Stanley, Parklawn, or Mayflower will get a meal pack from Burge Marketplace.

Residents in Hillcrest, Slater, Rienow, and Quad will get a meal pack from Hillcrest Marketplace.
